Please use the form below as part of the the Island Impact Camp 2024 application if the applicant is
under 13-17 years old AND is not attending with a parent or guardian.

…the camp leaders, in my absence and if I cannot be contacted, to obtain medical care and treatment, in the discretion of the leaders, that is deemed necessary for the health and well-being of the above child/ren (under 18 years of age) while part of the program. …the above child/ren (under 18 years of age) while part of the program to undertake travel deemed necessary as part of the program. …YWAM Pacific Waves staff and volunteer assistantsfrom any liability whatsoever arising out of illness, injury or death, damage or loss relating to the above child/ren which may be sustained by said persons during the course of their involvement with the program. I hereby authorise YWAM Pacific Waves to use any photos and/or video footage of the above child/ren (under 18 years of age) while part of the program for the purposes of promotion material. |
